This informed consent and liability waiver is written exclusively for lash artists certified in Opal Free™ by Lash Apothecary and using the Opal Free™ adhesive with the Series 2 Lamp.
Because Opal Free™ is an acrylate-free, cyanoacrylate-free adhesive system with biocompatibility and risk management evaluation (ISO 10993-1 & ISO 14971), this consent form is structured differently than traditional lash waivers while still clearly acknowledging that no cosmetic procedure is entirely risk-free.
Each section of this form is structured for client initials and includes a formal signature block for execution.
This document reinforces that Opal Free™ may only be applied and removed by Lash Apothecary–certified professionals and clearly outlines system-specific protocols.
